Video Footage & Performance Data Involving a Minor Player

This form is required before PIRlo Holding Company LLC ("PIRlo") may process match video footage or performance data involving a player under 18 years of age. Please read it carefully and complete all sections. Questions? Contact us at nancy@pirloanalytics.com.

3. What You Are Consenting To

By signing this form, you authorize PIRlo to:

• Receive and process match and training video footage in which the player appears, whether uploaded by you, the player's club or academy, or captured through an integrated camera system.
• Analyze that footage using computer vision technology that detects players, reads jersey numbers, and tracks on-field movement in order to measure individual performance. PIRlo does not use facial recognition.
• Process the player's basic profile information (name, age group, team, position, jersey number) and, where the club uses an integrated provider, GPS/wearable physical performance data.
• Generate individual performance assessments and reports, delivered only to you and/or the club, academy, or authorized agent that commissioned them.
• Maintain the player's performance records over time to support development tracking and recruitment opportunities, and use anonymized data (with names and identifying details removed) to improve PIRlo's statistical analysis.

4. How the Player's Data Is Protected

• The player's data is used only to produce the analytics services described above — never for advertising, and never sold or rented to third parties.
• Video footage is held only transiently while PIRlo processes it and verifies the resulting data, and is then purged as promptly as possible. PIRlo does not store match footage on an ongoing basis.
• Reports on minor players are never published publicly by PIRlo. Reports are shared only through links the account holder controls; the account holder may keep a link private, share it with specific recipients, or make it public, and may gate or revoke access at any time.
• You may withdraw this consent and request deletion of the player's identifiable data at any time by emailing nancy@pirloanalytics.com. Withdrawal does not affect processing completed before withdrawal, and anonymized data (which identifies no one) is not affected.

5. Required Consent (initial each line)

I am the parent or legal guardian of the player named above and have legal authority to provide this consent. I consent to PIRlo receiving and processing video footage and performance data involving the player, as described in Section 3, for the purpose of producing player assessments. I confirm that the footage submitted was lawfully recorded and that I have the right (or the club has the right) to share it with PIRlo. I have read and agree to PIRlo's Privacy Policy and Terms of Service.

7. Acknowledgment & Signature

I have read and understood this form. I provide this consent voluntarily and understand I may withdraw it at any time. This consent remains in effect until withdrawn in writing or until the player turns 18.
